**Note :

All paper work must be received by March 10th. Any paperwork not submitted by this date must be at the project for SRC review. Any paperwork received after March 10th will not be returned to the student and will be placed at the project site.
No student will be penalized for not submitting paper work before the fair but all paperwork for all students must be present at their project to be eligible for ISEF awards. The AzSEF is not responsible for any lost or destroyed paper and all students
should retain originals.

Applications are due by Midnight on Wednesday, March 5, 2008 . All accompanying paperwork is to be postmarked or faxed to the AzSEF Office by March 10, 2008.

Prior Approval Paperwork Reminder: If prior approval was required from the Scientific Review Committee or the Institutional Review Board (human subjects) students should have copies of all forms submitted to our office in his/her notebook. Do not call our office for copies.

Form 4 for human subjects must be presented during registration and interviews when requested.
We will re-review your project to make sure that you still comply with the SRC or IRB requirements.

The AzSEF Director will contact you or your teacher to discuss paperwork discrepancies and issues to be resolved prior to the event.
